Cigna 1500: $36.00 (60+ $45.00)

$1500 max for the year (coverage: cleanings - 100%, fillings (basic)- 20%, crowns, root canals, bridges (major)- 50%, limited ortho coverage)

Cigna 1000: $32.00 (60+ $40.00)

$1000 max for the year (coverage: cleanings - 100%, fillings (basic)- 20%, crowns, root canals, bridges (major)- 50%, no ortho coverage)

Cigna Preventive: $18.00 (cleanings only)

(coverage: cleanings only- 100% coverage)
